1. Position Summary
Encore Performing Arts is pleased to welcome the above-named youth as a Youth Set Assistant for the production of Seven Brides for Seven Brothers. This is a temporary, short-term role to assist in the construction and assembly of stage scenery under the supervision of adult staff.
2. Dates and Schedule
Work is expected to take place between:

Schedule and number of hours will vary depending on availability and production needs. Youth Assistants may not work more than 4 hours per day or 16 hours per week, in compliance with Utah labor laws for minors.
3. Compensation
A stipend of ____ per hour will be paid for all approved work, not to exceed ____ total. Payment will be made by check at the end of the assignment. This stipend is considered taxable income and may require a Form 1099 if earnings exceed $600 in the calendar year.
4. Supervision and Conduct
The Youth Set Assistant will work under the supervision of Encore staff and/or designated production team members. All participants must:

Wear appropriate clothing and protective gear

Treat all cast, crew, and volunteers with respect

Refrain from horseplay or dangerous behavior
5. Parental Consent
As the parent/guardian of the participant, I understand the nature of this role and give my consent for my child to participate. I understand that Encore will provide appropriate supervision and training and that reasonable precautions will be taken to ensure safety.
I acknowledge that participation is voluntary and that Encore Performing Arts is not responsible for injuries resulting from failure to follow safety instructions or misconduct.
6. Liability Waiver
By signing below, I agree to release and hold harmless Encore Performing Arts, its staff, board, and volunteers from any liability, claims, or damages arising from participation in this role.
